Young people and care leavers

We support young people aged between 16 and 25 by encouraging them to focus on their strengths and build positive support networks to achieve their aspirations and goals, whether that is returning to education, finding meaningful work or making new friendships.

Our services specialise in supporting young people facing homelessness, including those who are:

  • Care leavers or looked after children (over 16)
  • Overcoming addiction
  • Young parents
  • Experiencing family or relationship breakdown

Our services for young people include supported housing services, which provide a safe place for young people to live, learn basic life skill and make the transition to independent adulthood.

We also offer move-on services to help residents find and maintain their own independent accommodation, as well as floating support or community outreach services where we provide one-to-one or group support to young people in the community (who don’t live in our properties).

All support is tailored to each person’s individual needs, through a structured care and support plan, which uses the Young Person’s Star model to encourage each person to take control of their lives and work towards achieving their personal goals. Support is focused on eight areas:

Young people and care leavers

  • We support young people and care leavers to develop the skills and confidence they need to live independently.
  • They might have had problems at home, come directly from care or be escaping abuse, violence or exploitation. Others may have mental health needs, drug or alcohol issues or have offended in the past.
  • Whatever their background, we know that with the right support, they can go on to achieve great things.

Tailored care and support

  • At de-lightcareandsupport Services we believe that working closely with service users and in partnership with a variety of professional services is key in assisting the individuals placed in our care to reach their full potential. We have a highly detailed care planning system in place which is operated based on a thorough pre-placement assessment. As much information as possible about the service user is gathered to ensure we understand why the placement is being proposed and what the care needs are likely to be.
  • This way, we can develop with the service user, agreed targets and be more specific about the outcomes we want to see for them whilst being able to help service users address issues pertinent to their specific needs as reflected in their individual Care Support Plans.

Education and Training

At de-lightcareandsupport.com we believe the sky is the limit and that everyone should be given a chance to follow their dreams and aspirations. Part of the philosophy of Frontcare 247 Supported Living is that service users placed with us undertake community education, voluntary or apprenticeship training in an area of their interest in order to prepare them for independent living.

Semi-Independence Training

  • Part of the work we do is to support and assist the service user in developing skills and knowledge in preparation for moving into independence. The staff of de-lightcareandsupport Living will work towards positively developing individual qualities, which will give service users an opportunity for personal growth and development in a number of areas.
  • We also offer a transition to full independence training programme which is tailored to work with the service user based on their individual needs.
